Welcome to Parkdale Queen West CHC

Effective April 1, 2017, Parkdale Community Health Centre and Queen West – Central Toronto Community Health Centres have voluntarily merged to become one centre. Our new name is Parkdale Queen West Community Health Centre.

Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a community-based health service organization serving south-west Toronto. We offer a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming. We provide services at three main clinic locations, as well as within the community.

Parkdale Queen West CHC has a new name and logo highlighting the history of each founding organization, and maintains the shared principles and commitment to our clients and communities. Our three locations remain in operation, providing their current services and programs. The providers delivering those services remain the same. Clients are invited to access services in the same manner they have done previously.

Our Service Area

We offer services to people living in the area bordered by Bloor Street, Yonge Street, the Lakeshore, and Parkside Drive.

Service area restrictions do not apply for individuals who are homeless, Indigenous peoples, or those accessing our dental or harm reduction services. For non-OHIP insured clients, our boundaries extend north to Dupont Avenue.

New Clients

Parkdale Queen West CHC is not a walk-in clinic.
